17th September is World Patient Safety Day.
It is a global initiative to promote safer safer healthcare practices and minimise patient harm. Patient safety is a growing global public health challenge. It has been found that at least 50% of all patient harm is avoidable. Every interaction in the care-giving process from patient admission to discharge and care in the community can contribute to patient harm.

Drug, Healthcare and Patient Safety
Drug, Healthcare and Patient Safety is an international, peer reviewed, open access journal exploring patient safety issues in the healthcare continuum from diagnostic and screening interventions through to treatment, drug therapy and surgery.
The ​Journal of Patient Safety is dedicated to presenting research advances and field applications in every area of patient safety. While Journal of Patient Safety has a research emphasis, it also publishes articles describing near-miss opportunities, system modifications that are barriers to error, and the impact of regulatory changes on healthcare delivery.
The journal’s mission is to provide an international forum for healthcare professionals to report, discuss, debate, and critically review all aspects of care delivery that impose a risk on patient safety, with the ultimate goal of eliminating preventable adverse events and improving patient outcomes on a larger scale.
